Amputation Rehabilitation Fellowship:
Applying

The program functions under the auspices of VA Office of Academic Affiliations, Advanced Fellowship Program for Interprofessional Polytrauma and Traumatic Brain Injury Rehabilitation. There is no VA service obligation after completion of the fellowship program. However, fellows are encouraged to seek VA employment.

To apply for the VA Puget Sound Health Care System Advanced Fellowship in Amputee Rehabilitation, please submit the following:

  • Application for Health Professions Trainees, VA Form 10-2850d (http://www.va.gov/oaa/app-forms.asp)
  • Personal statement (1 page)
  • 3 letters of recommendation, including one from residency program director
  • Curriculum Vitae (3 pages or less)

Deadline for application is October 1 of the calendar year preceding the planned fellowship matriculation (i.e. October 1 of the PGY-4 year).
